Transition

 Harmony is a short-stay education provision designed to support young people tricky times in their education. We work closely with students, families, and professionals to help plan and support transitions back to a current school, into a new placement, or on to college. We will provide an in-depth report for reviews (as well as termly reports), detailing what we have found works for the young person - this can be used to help in decisions about future placements. With a wealth of experience and knowledge, we’re always happy to help identify the next right step and support each transition at a pace that feels safe, manageable, and appropriate for the young person.
